I've already set the clock to 12:00 and I've tried pulling the strings and clicking on the small door on the top. The Santa popped out with the puzzle piece in its hand but I couldn't get the piece.

You have to time it correctly and position the cursor correctly. Don't worry if you don't get it the first time. Contrary to what some may believe about needing to move the clock hands all the way around the clock if you mess up, all you have to do is pull the strings alternately 3 or 4 times to open the door again rather than again. Just keep trying and you'll get it eventually.
